NHS Digital - GP registered populations
Number of patients registered at GP Practice, from NHS Digital. Data is available for GP, Primary Care Network (PCN), Sustainability and transformation partnership (STP), Clinical Commissioning Group (CCG) and NHS England Commissioning Region level data are released in single year of age (SYOA) and 5-year age bands, both of which finish at 95+, split by gender.

Predicting demand for services and adult social care in Southampton analysis
In the context of an ageing population, increases in morbidity (ill health) and a reduction in resources, there is a need to better understand how this could impact on future demand for health and social care services in Southampton. Using the current prevalence of chronic health conditions, an analysis was undertaken in 2022 to model and project the number of Southampton residents with chronic health conditions in the future. An overview of this analysis, along with the results can be found in the slide set which can be downloaded below.
